About Supports for Health

Supports For Health (SFH) is a program developed to improve overall social, emotional and physical well-being by addressing social determinants of health. Differences in social determinants of health often contribute to the disparities in chronic disease prevention and treatment which are prevalent across our country among differing racial, ethnic, and socioeconomic groups, limiting opportunities for members of some of these groups to achieve and maintain health.

Improving Food Security

Increasing food security and nutrition through a Food as Medicine / Produce Prescription Program that offers nutrition counseling, subsidized food and cooking classes with the goal of improving access to and consumption of nutrient dense fresh produce.

Health Education Classes

Improving lifestyle choices related to nutrition and physical activity with chronic disease education and support services provided through Chronic Disease Self-Management Program (CDSMP) and National Diabetes Prevention Program (NDPP) and Community Health Worker support.

Access to Transportation

Addressing transportation obstacles that negatively affect health, including access to grocery shopping and food pantries by providing subsidized transportation and long-term solution-oriented planning and referrals for transportation needs.

Who qualifies for Supports for Health?

Supports for Health is a program designed for Cortland County residents who are low-income individuals and/or families and/or immigrants and migrant workers. Each program component has its own eligibility criteria.

National Diabetes Prevention Program

This year-long lifestyle change program is aimed at decreasing an individual's chance of developing type 2 diabetes through education and healthy lifestyle skills. This program uses the CDC National Diabetes Prevention Program curriculum and a lifestyle coach to help you create and achieve your health and weight loss goals.

Chronic Disease Self-Management Class

We know that living with a chronic condition or caring for someone with a chronic condition is hard. If you or a loved one has chronic pain or a chronic health condition and are looking for ways to live a healthier life, let us help you with this 6-week class designed to teach you how to take back some control and start living a more positive, healthier life.

Transportation Assistance

The Supports-for-Health program will provide subsidized transportation for eligible recipients to destinations and services essential to their health not eligible for Medicaid rides. Eligible trips include but are not limited to: access to food, prescription medication pick up, renewal of services (e.g. SNAP), chronic disease education class, physical exercise class, or other supports essential to maintaining good health. Program participants are NOT required to be a Medicaid recipient to be eligible.

Produce Prescription Program (PRx)

Our Produce Prescription Program (PRx) is one component of our Supports for Health program, using a ‘food as medicine mode’ to increase positive health outcomes by improving food security and preventing/treating diet-related chronic diseases by increasing access to and consumption of more fresh produce for those in Cortland County.
